Registration fees* for Fall Season: $85.00
There will be a $10 discount for Early Registration
May 1, 2009- June 15, 2009
(payments must be postmarked before June 15th to keep discounted rate)
*Registration fee includes uniform (jersey, shorts, and socks)
REGISTRATION REQUIREMENTS FOR FALL SEASON 2009
ü Players must be 4 years old on or before July 31, 2009 to register for Fall Soccer
ü Follow registration directions posted on website. Select the appropriate age group assignment.
ü Players new to the club MUST provide a COPY of a "State
Certification of Live Birth" with affixed State seal or certification with

If a new player’s registration does not include the appropriate documentation, it will be returned without processing. No exceptions will be made.
**This is a Florida Youth Soccer Association rule and IYSA must comply
ü Registration fees must be received to process registration. If registration fees are not received, the child will not be placed on the team.
OPEN REGISTRATION CLOSES JULY 15th. AFTER THAT DATE REGISTRATIONS WILL BE TAKEN ON A SPACE AVAILABLE BASIS.
